Four compounds, four distinct mechanisms.
Tirzepatide, Cagrilintide, NAD+ and SS-31 — incretin signalling, amylin satiety, mitochondrial co-enzyme supply, and cardiolipin structure. Four separate research questions, available individually or in one order.
What each compound is studied for.
Incretin receptor pharmacology, amylin signalling, mitochondrial co-enzyme availability and inner-membrane structure are four separate literatures, and they are not equally well supported. Tirzepatide has the most mature clinical record of the four. Cagrilintide has published phase 2 work behind it. NAD+ rests largely on established biochemistry, with human precursor studies that are mostly small and short. SS-31 has been studied mainly in rare mitochondrial disease populations, which does not generalise to healthy physiology. Grouping the four is a purchasing convenience. Nothing here establishes that they should be used together, and we do not publish guidance on combining compounds.
What’s in this stack.
Dual GIP and GLP-1 receptor agonist — one peptide that binds both incretin receptors. Published clinical trials report effects on glycaemic and body-composition endpoints. Those trials used pharmaceutical-grade material under medical supervision, and are cited here as background on the molecule rather than as a description of this product.
Long-acting amylin analogue. Amylin is co-secreted with insulin at meals and signals satiety through area postrema pathways. Cagrilintide has published phase 2 work behind it; PTW makes no claim about its use alongside any other compound or medicine.
Cellular energy co-enzyme. Required for mitochondrial electron transport chain function and ATP production, and the substrate the sirtuin enzymes depend on. Levels decline with age.
Mitochondria-targeted tetrapeptide. Concentrates in the inner mitochondrial membrane and binds cardiolipin, the phospholipid that scaffolds the respiratory chain. Studied for whether preserving that structure preserves electron transport efficiency under oxidative stress.
What the published work describes.
Tirzepatide — Dual Incretin Pathway Activation
A synthetic peptide that co-activates GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 receptors. The two receptors are distributed differently through the body: GIP receptor activity has been studied at adipose tissue and insulin sensitivity, GLP-1 receptor activity at appetite signalling, gastric emptying and hepatic glucose output. Whether engaging both produces something the mono-agonists do not is the open question the literature is still working through.
Cagrilintide — Amylin-Pathway Satiety Extension
A long-acting amylin receptor agonist. Amylin is co-secreted with insulin at meals and signals satiety through area postrema and nucleus tractus solitarius pathways, independent of GLP-1. Its extended half-life is what the published phase 2 work examines. PTW makes no claim about its use alongside any other compound or medicine.
NAD+ — Role in Oxidative Phosphorylation
Nicotinamide adenine dinucleotide is the primary co-enzyme in mitochondrial oxidative phosphorylation. NAD+ levels decline with age, and are consumed faster under oxidative stress. Depletion is associated in the literature with reduced fat oxidation efficiency, lower ATP output and limited sirtuin activity. Whether supplementation changes any of that in a person is a separate question from the biochemistry, and a far less settled one.
